Default NVIDIA/Old STB ISA Video Card (FCC ID: EKSUSAPGCOMBO)

I'm hoping someone might have documentation on the various jumper settings
(J1-J6, J7, J8-J20) for this old card. (Dated 8/1992).

The FCC registration info at

(FCC ID: EKSUSAPGCOMBO) says it's a "VGA Video Card, Maximum resolution 1024
x 768. Card has parallel port, two serial ports, has mouse port and IDE
disk controller built in."
