See if card is seated well in PCI slot, could be that motherboard
is misaligned in case so some contacts are shorting, or card
and/or board has defective contacts or slot.

Or it could be that card isn't compatible, a rare situation but
it happens. Try card in another system but beware if card is
damaged.

Another potential issue might be that your power supply has
insufficient 5VSB capacity. If it's a generic PSU or rated for
less than 1.5A (more or less depending on other cards and USB/PS2
option for 5VSB power) you might try another PSU.
