From your dscription of what you have done, it sound like [my guess] your
card need to make an appointment with nVidia cementary.
During the meantime, see if your card can function with lower "Hardware
acceleration" in TroubleShooting tap. If you have "Enable wirte combining"
checked, try to uncheck and see if it work without those freeze or obnormal
issue. You might also can try to under clock your card and see if it work
better while deciding what to do next.

well i've tried underclocking it already and that didn't seem to affect
it and i suspect the other settings won't have an effect as sometimes
the lines appear over the logo at boot-up and in safe mode.

Probably best to RMA it and hope the company doesn't take forever to
send out a replacement
